Form DS-11 is the Application for a U.S. Passport, issued by the U.S. Department of State. It's used by people who must apply in person — most often first-time applicants and children. You fill it out ahead of time, then submit it at an acceptance facility.

You must apply in person with Form DS-11 if any of these apply to you:
Use the DS-11 when you must apply in person (first passport, child, or a passport that's too old or was lost). Use the DS-82 when you're eligible to renew by mail — generally if your most recent passport is undamaged, was issued in the last 15 years, and you were 16 or older when it was issued.
Do not sign the DS-11 in advance. You must sign it in person in front of an acceptance agent, or your application will be rejected.
Submit your DS-11 in person at a passport acceptance facility — often a post office, library or clerk of court — or at a regional passport agency. Many locations require an appointment, so check ahead. Do not mail a DS-11 on your own.
A passport involves two fees: an application fee paid to the Department of State, and an acceptance (execution) fee paid to the facility. Costs differ for a book, a card, or both, and for adults vs. children. Processing times vary, with an expedited option for an extra fee. Check the current fees and timelines on travel.state.gov before you go.
