What is Form DS-82?

Form DS-82 is the U.S. Passport Renewal Application you can submit by mail — no in-person appointment required. If you qualify, you fill it out, sign it, and mail it in along with your most recent passport.

You can complete every field here and print a clean, ready-to-mail copy.

Can I renew by mail with the DS-82?

You can use the DS-82 if your most recent passport meets all of these:

  • It's undamaged and you can submit it with your application.
  • It was issued when you were age 16 or older.
  • It was issued within the last 15 years.
  • It was issued in your current name — or you can document your name change.

If any of these don't apply, you'll need to apply in person with Form DS-11 instead.

DS-82 vs DS-11 — which do I use?

Use the DS-82 to renew by mail when you qualify under the rules above. Use the DS-11 when you must apply in person — for a first passport, a child under 16, or a passport that's lost, damaged, or more than 15 years old.

What you'll need to fill it out

  • Your full name, date and place of birth, and Social Security number.
  • Your mailing address and contact details.
  • Your most recent passport — number, issue date, and the name it was issued in.
  • One recent passport photo (2x2 inches).

How to fill out Form DS-82

  1. Open the form in the editor — the official renewal application loads ready to type into.
  2. Enter your name and birth details exactly as they appear on your current passport.
  3. Add your address, SSN and contact info.
  4. Enter your most recent passport number, issue date and name.
  5. Sign and date the application, then download and print it to mail in.

What to mail in

  • Your completed, signed DS-82.
  • Your most recent passport (it will be returned separately).
  • One 2x2-inch passport photo, taken within the last 6 months.
  • Payment for the renewal fee — by check or money order.
  • Any name-change documents, if applicable.

Where to mail your DS-82

Mail your application to the address listed on the official form for your state and service level (routine or expedited). The mailing address differs by region, so use the current address on travel.state.gov. Consider a trackable mail service since you're sending your passport.

Fees & processing

Renewing by mail involves the passport fee only — there's no separate acceptance fee since you're not applying in person. Costs differ for a book, a card, or both. An expedited option is available for an extra fee. Check current fees and processing times on travel.state.gov before mailing.

Signing your DS-82

Unlike the in-person DS-11, you sign the DS-82 yourself before mailing it — no acceptance agent required. Date it the day you mail it. You can type, draw or upload your signature here, then print the signed copy.

"Renewed without taking time off."

My passport was expiring before a trip. Filled out the DS-82 at my desk, signed it, and mailed it with my old one. No appointment, no post-office line.

"So much easier than handwriting."

I typed everything, double-checked my old passport number, and printed a clean copy. Way less stressful than trying to write neatly in tiny boxes.

"Clear on what to mail."

The guide spelled out exactly what to include — the form, my old passport, a photo and the fee. Dropped it in the mail and had the new one in a few weeks.
